Cardioversión eléctrica electiva en hospital de día
Introducción. La fibrilación auricular (FA) es la arritmia más frecuente. La cardioversión eléctrica y farmacológica se indica en aquellos pacientes sintomáticos con FA persistente. En nuestro centro las cardioversiones eléctricas electivas (CVEE) se realizan en el hospital de día de cardiología. Un elevado número de ellas son anuladas. Objetivos. Describir las características del paciente que acude para CVEE y los motivos de no realización de la misma. Material y métodos. Estudio prospectivo observacional que incluye a todos los pacientes que acudieron al hospital de día para una CVEE entre enero y julio de 2015. Variables recogidas: socio-demográficas, clínicas, tipo y tiempo de anticoagulación, efectividad de la CVEE o, causas de no realización de la CVEE. Resultados. Se analizaron 97 CVEE (77,3% hombres y edad media de 62±14 años). El 45,4% con CVEE previa. La arritmia más frecuente con 93,8% fue la FA. El 63,9% tomaban acenocumarol y tenían un INR de media de 2,74±1,25. La cardioversión se realizó en un 57,7%, siendo exitosa en un 80,4%. Las causas de no realización fueron: INR elevado (11,6%) y bajo (32,6%), tratamiento incompleto (2,3%) y ritmo sinusal (53,5%). Conclusiones. En un 43,3% de los pacientes la CVEE fue anulada, mayoritariamente por estar en ritmo sinusal, seguido de INR no óptimo. Como medida de mejora para reducir el número de anulaciones y visitas innecesarias, proponemos contacto previo con la enfermera para educación al paciente en la toma de pulso radial e identificación del ritmo cardiaco, recordándole cita, revisando tiempo de anticoagulación y valores de INR
Elective electrical cardioversion in the outpatient clinic
Introduction. Atrial fibrillation (AF) is the most frequent arrhythmia. Electrical and pharmacological cardioversion is indicated in those symptomatic patients with persistent AF. In our center, elective electrical cardioversions (EECV) are performed at the cardiology outpatient clinic. A high number of them are canceled. Objectives: To describe the characteristics of the patient who comes for an EECV and the reasons for not performing it. Material and methods. Observational prospective study that includes all patients who went to the outpatient clinic for an EECV between January and July 2015. Collected variables: socio-demographic and clinical variables, anticoagulation type and time, effectiveness of EECV or causes for not performing the EECV. Results. 97 EECV were analyzed (77.3% male and mean age 62 ± 14 years). 45.4% of them had a previous EECV. The most frequent arrhythmia was AF (93.8%). 63.9% were taking acenocoumarol and had a mean INR of 2.74 ± 1.25. Cardioversion was performed in 57.7% of them, being successful in 80.4% of cases. The causes for not performing it were: high (11.6%) and low (32.6%) INR, incomplete treatment (2.3%) and sinus rhythm (53.5%). Conclusions. In 43.3% of the patients EECV was canceled, mostly because they were in sinus rhythm, followed by non-optimal INR. As an improvement measure to reduce the number of cancellations and unnecessary visits, we propose previous contact with the nurse to educate the patient in taking radial pulse and identifying heart rhythm, reminding him of the appointment, checking anticoagulation time and INR values
